Forms to Download (AY2026)

For Regular (First-Year) applicants: Share this form with your counselor or another high school official responsible for documenting student academic progress and ask them to complete and submit (upload) it. This report shares supplementary information regarding your academic achievements and school details.

For Transfer (Second-Year) applicants: Ask an official at your current college/university to complete this form. This form certifies that you are either enrolled at the institution as a bachelor degree-seeking student (not as a non-degree-seeking student) or expected to obtain an associate's degree by the time of enrollment at ICU.
For One-Year-Regular applicants (OYR) and Kenkyusei (Special Students): Ask an official at your current (or most recently attended) college/university to complete this form. This form certifies that you meet the eligibility criteria for the desired student status.

For Recommenders: If you have technical difficulties using the submission portal and wish to email your letter of recommendation directly to the ICU Admissions Center, please attach this information sheet. If using the submission portal, you do not have to use this sheet as you will be asked to complete the same information online when uploading your letter of recommendation.

For IB Diploma Candidates: Ask your IB Coordinator to complete this form. If the school has its own report form for this purpose, the IB Coordinator may use it and does not need to use this form.
